Job Title: SOCIAL WORK CASE MANAGER
Reports to: Social Work Case Manager
Hours: Full Time
Compensation: $2167.00 to $2500.00 depending on experience

Duties and Responsibilities
The Social Worker is responsible for case management services for the supportive housing programs and properties administered by Samaritan House. Responsibilities include but are not limited to: psychosocial and needs assessment, development of comprehensive care plans, coordinating services with other collaborative agencies, and client advocacy within the community as necessary. The Social Worker will be a part of a multidisciplinary team, maintaining confidential client files, and tracking all units of service provided.
Qualifications
v Minimum of a Bachelor’s degree in Social Work. Must be licensed by the State of Texas to practice Social Work.
v Case Management Experience in public and/or assisted housing environment or other social service agency.
v Knowledge of social service principles, practices, and techniques
v Ability to develop, implement, and monitor care plans for individual resident’s needs.
v Knowledge of and ability to adhere to the Social Work Code of Ethics
v Basic skills for the operation of office machines, computers and computer programs.
v Ability to effectively communicate with staff, residents and administrative personnel from varied cultural and socio-economic groups.
v Knowledge and sensitivity to HIV/AIDS issues, cultural/ethnic issues and diverse populations, gay and lesbian issues, chemical dependency issues, and grief and bereavement.
v Bilingual a plus.
v Current Texas Driver’s License.
v CPR Certification (within 6 months)
v Ability to pass a pre-employment drug screening and background check.
