Senior Citizen Services announces staff opening: Health Education-Chronic Disease Coordinator, PT
Coordinates and administers Chronic Disease Self Management (CDSM) and Diabetes Self Management (DSM) Programs which are designed to help people gain self-confidence in their ability to control their chronic disease symptoms and how their health problems affect their lives. Part-Time Position: 29 hours per week.
Basic functions of the position are:
Requirements: College degree required. Degree in Public Health, Science, Gerontology, Physical Education, Fitness, Social Work with medical emphasis, or related field desirable. Knowledge of aging process and experience in the field of aging, health and fitness, and teaching skills and experience. Experience with chronic disease management and small group facilitation preferred.
