Senior Citizen Services announces staff opening:  Health Education-Chronic Disease Facilitator, PT

Assists the Chronic Disease Self Management Coordinator in the delivery of the Chronic Disease Self Management (CDSM) and Diabetes Self Management (DSM)  Programs which are designed to help people gain self-confidence in their ability to control their chronic disease symptoms and how their health problems affect their lives.  Part Time Position 15 hours per week.

Basic functions of the position are:

• In conjunction with the Program Coordinator, or other personnel designated by the coordinator, conducts regular workshops in CDSMP/DSMP which strictly comply with the program as written in the Stanford University Medical School Leader’s Manual.
   
• Assists Coordinator in applying appropriate volunteer administration to recruit and train program coaches and other volunteers.
   
• Conducts program workshops in accordance with specific program requirements and assists in the implementation of the organizational structure, data collection, reports, and production of workshop manual and materials, and supports the protocols of evidence based programs.
   
• Must possess or complete Stanford School of Medicine training as a CDSMP/DSMP Leader.  Must maintain annual certification by attending yearly update with healthcare professionals and other CDSMP/DSMP Leaders.

Requirements: College degree preferred. Degree in Health, Science, Gerontology, Physical Education, Fitness, Social Work with medical emphasis helpful.  Knowledge of aging process and experience in the field of aging, health and fitness, and teaching skills and experience.  Experience in chronic disease management and small group facilitation preferred.
